You maybe wonder what it is that transition means? Transition is name we give to the time in your life when you are growing up, leaving school, going to college, maybe finding a job and growing into a young adult. Transition covers all of these things and more.

Our Transition service is setup to help you do all of these things and handle all the changes that life throws at you in the transition to adulthood.

The Transition Team start working with people whilst they are still in school. They usually meet people when they are in year 9 of school, however if you are older than that it does not matter.

From there they can help you plan what you would like to do in the future, from deciding your GCSEs, what you want to do after school, plan if you want to go into higher education or work based learning right through to work opportunities to help develop a plan for your early adult life.

The team can also help you look at money, transport, living and leisure arrangements as part of your future plan as well.
