Person Centred Approaches and Working

Person-centred care is at the heart of all Health & Social Care Services.

A good service (whether health, social care, education, employment, public or private sector), makes sure the person is at the centre of decision making – remaining in control of the decisions that affect their life, and where required, support given to them to ensure decisions made are in the best interest of that person.

Plans made to support the person need to recognise and identify what is most important to and for the person. The person should be given relevant and up to date information and advice to make sure they choose what is best for them.

Families and Carers are essential to people with learning disabilities: it is important they are recognised and supported in their vital role.

There are numerous person centred tools to help individuals and families to identify what’s best for them.

What If – Celebrating My Life Plan

t the appropriate times this plan is to enable People and their love ones, to make advanced decisions: regarding the person’s wishes in the distant future, and make the most of the present.

What If - asks key questions to think about:

  • The importance of making a will
  • Are there other people involved in making decisions for the person (e.g. Is there a Lasting Power of Attorney in place?)
  • Where does the person wish to spend their final days?
  • How does the person want their life to be celebrated: and their final messages to loved ones?

What If: in an informal way begins the conversation and reminds us: “Don’t leave it too late to say what you want”

Where advanced concerns over clinical care are raised these will be raised with the relevant Health professionals involved at the time.
