Health & Wellbeing Co-ordinators

The Care Plus Health & Wellbeing Coordinators are a dedicated team that help to organise your care and looking after you. You can find more information on some of the things we do by following these links.

However it is important to understand that this is not all they do.  Once you are working with a Health & Wellbeing Coordinator and you have any questions or concerns about your health, support, care, home life or general questions they are here to help. They are here to talk to you and support you with anything you want to talk about.

They will also help you to attend appointmnets such as Doctors, Hospital, Dentist and Opticians should you need more help. We also run some training course for people with a learning disability around womens health, mens health, sexual awareness, diabetes, keeping safe in the community, eating healthily and lots more.

 Meet The Team

We currently have 2 Health & Wellbeing Coordinators in the team.  Which one you get to work with depends upon who your doctor is, but you must live in the North East Lincolnshire around Grimsby, Cleethorpes and Immingham in order to get a Health & Wellbeing Coordinator.
